Privacy Notice

What we collect, why we hold it, who ever sees it, and what you can ask us to do with it.

Last updated 2 September 2026

Soulistic Clinic keeps health records. We treat that as the serious thing it is. This notice explains, in plain language, exactly what we hold and what you can ask of us. It is written to meet the Data Privacy Act of 2012 (Republic Act No. 10173) and the rules of the National Privacy Commission.

In short. We collect what we need to care for you and nothing else. Your health information is never sold, never used for advertising, and never shared except as described below. You can ask to see it, correct it, get a copy, or have it deleted, and we will answer.

1. Who is responsible for your information

Soulistic Clinic is the Personal Information Controller for the information described here. That means we decide what is collected and why, and we are accountable for it.

For any question or request about your information, contact us:

Requests about your own information are handled by the clinic directly. If you would like your request routed to whoever is designated to handle data protection matters, say so and we will make sure it reaches them.

2. What we collect, and when

We only collect through the forms you fill in and the conversations you start. We do not buy information about you from anyone.

When youWe collect
Fill in the New Patient Form Your name, date of birth, age, sex, marital status and occupation. Your address and contact details. Your health background, which includes whether you take prescribed or other medication, whether you have a contagious condition, a recent injury, a psychiatric condition or a diagnosed illness, and the symptoms you are experiencing. What brings you in, where you feel it, and how strongly, on the scales in the form. Your signature, drawn on the screen. How you heard about us.
Book a session or a discovery call Your name, phone number, email address, the kind of session you want, your preferred date or chosen time slot, and anything you write in the notes.
Pay for a session The reference number of your payment and, if you send one, the screenshot of your receipt. We do not see or store your GCash login, card numbers or bank credentials. Payment is made directly to the clinic's own account.
Register for a course, retreat or webinar Your name and nickname, date of birth, address, contact details, profession and company, how you heard about it or who invited you, your religious affiliation where the course asks for it, your signature, and any certificate issued to you.
Send a testimonial Your name, your company if you give one, your email address and what you wrote.
Ask about corporate wellness Your name and position, your company and its details, and what you tell us about what your organisation needs.
Chat with Ruby on this site What you type and what Ruby replies, so the clinic can see what people are asking and improve the answers. Do not put anything in the chat you would not want on file. If you need to tell us something private, use the form or call us.
Simply visit the site We keep a count of visits and which pages were opened, to know whether the site is useful. We do not use advertising trackers and we do not build a profile of you.

3. Sensitive personal information

The law treats some of what we hold as sensitive personal information, which carries stronger protection. In our case that means your health, your age and date of birth, your marital status, and, where a course asks for it, your religious affiliation.

We collect these because a healing session cannot be given safely without knowing them, and because the Foundation's course records ask for them. We rely on your consent, which you give by ticking the box on the form before you submit it. You can withdraw that consent at any time by writing to us, and we will stop using your information, subject to the records we are required to keep.

4. Why we hold it

We do not use your health information to advertise to you. If you have asked to hear from the clinic, you may receive occasional messages about sessions, courses or offerings, and every one of them will let you stop with a single reply or click.

5. Who else ever sees it

Your information stays with the clinic, except in these cases:

We never sell your information, and we never share it for anyone else's advertising.

6. How long we keep it

Patient records are kept for as long as you remain a patient and afterwards for as long as the clinic needs them as a record of care given. Course records are kept for as long as the Foundation's certification requires. Booking enquiries that never become sessions, and chat conversations, are kept only as long as they are useful and then removed.

If you ask us to delete your information we will do so, unless we are required to keep a particular record, in which case we will tell you which one and why.

7. How it is protected

No system is perfect. If a breach ever puts your sensitive personal information at real risk, we will notify you and the National Privacy Commission as the law requires, and tell you plainly what happened and what we are doing about it.

8. Your rights

Under the Data Privacy Act you have the right to:

To use any of these, write to hello@soulisticclinic.ph. We will confirm who you are, then answer. We aim to respond within fifteen working days, and we will tell you if a request needs longer.

9. Children

Where a patient is a minor, the form is completed with the knowledge and consent of a parent or guardian, and their information is treated with the same protection described here.

10. Changes to this notice

If we change how we handle your information we will update this page and change the date at the top. If a change is significant, we will say so rather than let it pass quietly.

Questions. If anything here is unclear, or you want to know exactly what we hold about you, email hello@soulisticclinic.ph or call +63 917 522 2084. You will get a real answer from a person.
